Contractor to repair sidewalk correcting pedestrian slope from 4% to 2%. Proposed work area is in a designated CRITICAL ARTERIAL roadway. No traffic control devices may be set if another lane closure is already set within 1500 feet upstream of the address noted on this permit. Contractor must notify 311 minimum three (3) days before planned lane closure. Provide 311 agent the Right-of-way Management permit number (ending EX or RW) work begin- and end-dates and times number and direction of lanes to be closed and reason for lane closure.
