Demo old driveway. Construct new driveway approach of 12 FT Good morning I have started the process of reactivating this permit. As per my managers confirmation we will not need any communication from the DSD inspector to get this permit extended just the approval from Sam West. He will be working with you regarding the Type III driveway waiver. Waiver To get the type 3 waiver you need to follow below instructions/steps- Apply to Approval from Land Development Engineering (LDE)(Sam West etc.). Authorized Agent Form/ One Time Authorization Ms. Moira Longino- [email protected] is not an authorized agent for the ROW contractor J Torres Construction. ROW contractor can either add them as authorized agent via AAF form or can provide a one time authorization from ROW contractors email address. Let me know your plan. Few more details which will require confirmation- 1. Please confirm the closure details for the work zone will require no change and we will keep Standard Detail 804S-4 sheet 3 (top half) behind the curb closure ONLY; staging from private property for the work zone. 2.
